In case of danger, there is a bell for her to ring.

Believing Leonora dead, Alvaro has joined the Spanish army under an assumed name. He hears a call for help and, running to the rescue, he finds Don Carlo, who has been attacked by a gang. Neither knowing who the other really is, they swear eternal comradeship. In the ensuing battle Alvaro is badly wounded and Carlo helps him to safety, where Alvaro hands him some papers that must be destroyed should he die. Carlo becomes suspicious of Alvaro´s identity and, after finding a portrait of his sister in his pocket, he challenges Alvaro to aduel. Alvaro reminds him of their oath, but Carlo is implacable. A passing patrol manages to separate them: Carlo is draggedaway and Alvaro enters a monastery - the Madonna degli Angeli.

Act IV

Five years on, Alvaro, now known as Father Raphael, lives a life of fasting and self-denial. Carlo enters the monastery and provokes a fight with Alvaro, during which carlo is mortally wounded. Alvaro rushes to the local hermit for absolution, at which point he and Leonora recognize each other. When he tells her of her wounded brother she rushes to Carlo´s side, but as she leans over to attend to his wounds, Carlo stabs her and dies. Alvaro curses heaven for having brought such pain to their lives. Believing that she and Alvaro will one day be reunited in heaven, Leonora dies - thus fulfilling the curse with which the opera began.
